In this session we will explore the importance of personalizing the social determinants of health (SDoH) and understanding an individual and communities lived experience. Acknowledging this concept is key in the pursuit of health equity, especially for marginalized communities. A number of interventions have been proposed over the years prioritizing lived experience and partnerships to achieve measurable impact.??
