Pharmacists
The Ambulatory Care Pharmacy Forum of North Carolina is a network for and led by ambulatory clinical pharmacists with an ongoing goal of providing networking, collaboration, and educational opportunities for likeminded pharmacists in North Carolina with an emphasis on practical ambulatory care topics and unmet public health needs.
In addition to providing networking opportunities for ambulatory care pharmacist, this application-based CPE activity will help pharmacists develop patient care skills for patients with cardiometabolic conditions (e.g. diabetes, hypertension, hyperlipidemia) through facilitated, interactive case discussions. Participants will interactively work through complex cardiometabolic patient cases in small groups and collective results discussed as a whole with advanced pharmacy practitioner experts.
